The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Your account is not satisfied until your check clears the bank.  If your check is returned unpaid, you agree to pay a returned check fee for the maximum amount allowed by the state. All balances that reach 120 days in arrears will be sent to a collection agency and you will be financially responsible for all fees incurred.  

Dental Insurance

We participate in several dental insurance companies.  We ask that you carefully read your policy to be sure you are fully aware of any restrictions that apply to the benefits provided.  Dental insurance is a contract between the patient and the insurance company for reimbursing the costs of dental insurance.  It is not a contract between the Dentist and the insurance company.  If we do participate with your insurance, all services will be submitted to your carrier for you unless we have received notification of non-covered services. We will be collecting all applicable co-pays, deductibles, co-insurance, and fees for non-covered services at the time of the visit.

If your insurance is not listed above, please call and ask if we participate. If we do not participate with your insurance, payment is expected at the time of service.  We will submit all claims forms to your carrier as a courtesy, without a charge, so that you may receive reimbursement.
